Necessary Paper Work
(Must have for ALL files)
• Last 2 years “full” income taxes including all
W-2’s/1099’s
• Two consecutive months of bank statements for
each bank account. Each must include your full name, address, and account
number
• Current pay-stubs showing the YTD info
• Latest statements for all investment/retirement
account (401K’s, IRA’s, Stock and Bonds, etc)
• Declarations page of your homeowner’s insurance
or name and phone # of your agent
• Copy of Driver License and Social Security cards

Miscellaneous Paperwork
(What ever applies to your situation)
• Copy of Divorce Decree
• Retirement Income: full rental contacts and
addresses of properties with all their mortgage information
• Bankruptcy: Copy of complete BK and Discharge
papers
• Co-signed loans: Letter from co-signer stating
responsibility for payments and 6 months cancelled check showing them making
payments
• Tax Liens: Current payoff statements or letter
that they will subordinate and a payment schedule
• Condos/ PUD’s: Homeowner’s Association name and
phone number
•Alien Resident: Copy of front and back of Green
Card
• Sale of current home: Contract, Settlement
statement when received
• New Construction: plans
• Current Second Mortgage: Copy of
Note/Installment and profit and Loss statement
• Self Employed: 2 yrs “full” business taxes,
Financial Statement and Profit and Loss Statement
• Stated Income: 2 yrs Business License
